Topical tranexamic acid is used in cosmetic serums for the look of melasma and uneven tone, but the evidence is limited. In one small 12-week split-face trial, a 5% gel was no better than its vehicle and caused more redness.

Topical evidence is limited. In a 23-person split-face trial, 5% tranexamic acid gel was no better than its vehicle after 12 weeks and caused more redness. A 22-trial review mixed topical, injected, and oral routes and found high heterogeneity. A separate 99-person study compared tranexamic-acid and niacinamide combination creams with 4% hydroquinone; it did not isolate tranexamic acid or prove the formulas equivalent.

J Cosmet Laser Ther, 2012Systematic review and meta-analysis of 22 randomised trials with 1,280 patients. Oral treatment showed the largest pooled MASI decrease, followed by injections and topical applications; studies had high heterogeneity, especially among combination treatments.
