A material disagreement stays visible. The record uses Controversial unless a stronger, relevant synthesis resolves it. A convenient source never cancels a credible conflicting one.

Moderate
Useful human evidence supports the claim, with a meaningful limit in scale, duration, formulation or consistency.
Limited
The record is early, small, indirect, mixed or tightly tied to one formulation.
Controversial
Credible sources point in materially different directions without a stronger synthesis resolving them.
Unknown
No gradeable evidence record supports this claim yet.
How records change
Each record carries a permanent ID and publication date. A later reviewed date appears only after the claim, grade, source set and caveat have been reviewed together. A renamed ID requires a version change and a redirect from the old address.
What a grade does not say
The grade belongs to the stated claim, route and studied formulation. It does not automatically transfer to another molecule, strength, product or outcome.
